Eligibility
Hospice programs and services meet a variety of support needs for individuals, families and care providers. Each hospice organization provides a mix of services based on community demand and the availability of resources and volunteers. Any individual, regardless of age or medical condition, can access hospice support once they have been diagnosed as being palliative. Members of their families or others who have suffered a loss can also access complementary supports where available.
Individuals may be referred by a physician or may request services directly on their own by contacting the hospice of their choice. Generally, those wishing to access hospice programs will receive them from a local organization, close to home. Each hospice is assigned a service area based on Ministry of Health and Long Term Care guidelines, however some offer services across these boundaries. In addition, individuals with urgent needs can be eligible to receive services wherever there is space available.
