Where to get care

Hospices build a bridge across health care services, offering quality end-of-life care that traditional services cannot provide in isolation. Each hospice organization provides a mix of services based on community demand and the availability of resources and volunteers.  

Normally, you would access hospice care close to home.  However, depending on your specific needs, you may be referred to another hospice or service provider.

Use your postal code to find the Community Care Access Centre near you.  The CCAC's are a resource provided by the Ontario Ministry of Health and Long-Term Care to help coordinate community-based health care services.
